PAYMENT WILL ALSO BE THRU FAMILY ID - REVTRAK PAYMENT. PAYMENTS ARE DUE ONCE A STUDENT HAS BEEN SELECTED FOR A TEAM. PAYMENT MUST BE MADE PRIOR TO THE FIRST GAME AND/OR MEET. Please be sure to include student name on Revtrak payment form.

ALL REQUIRED FORMS ARE SPECIFIED IN THE REGISTRATION SIGN-UP WITH FAMILY ID, INCLUDING AN UP-TO-DATE PHYSICAL THAT MUST BE ON FILE WITH OUR SCHOOL NURSE. STUDENTS WILL NOT BE ALLOWED TO TRYOUT OR PRACTICE IF REGISTRATION FORM IS NOT COMPLETED.
TYPICALLY STUDENT ATHLETES SHOULD PLAN TO COMMIT TO 5 DAYS PER WEEK FOR THEIR SPORT - INCLUDING PRACTICES AND GAMES/MEETS.

Health Office Important Reminders - state mandate:
To All Athletes: Students who intend to try out for inter scholastic school sports - i.e. cross country, district football team, soccer, basketball, cheerleading, field hockey, softball, baseball and track need to have a current physical examination report on file in the school nurses' office. The updated physical examination report must be complete and dated by the physician within 13 months of the beginning of the sport's season.
Students cannot tryout for any sports team without this report being on file at school. If a physical expires during the sport's season, your child CANNOT continue to play until a new physical has been completed by your physician and turned into the health office.
